6. Acceptance Form (SIGNED BY CANDIDATE).
7. Online Clearance Eligibility Form (DULY ENDORSED BY ADMISSIONS OFFICER).
8. Birth Certificate or Sworn Declaration of Age.
9. Copy of Health Centre Registration Card.
10. Guarantors Form (SIGNED BY THE GUARANTOR).
11. Local Government Certificate of Origin OR Identification Letter.
12. Reference Letter From Two Referees.
13. Online Personal Data Form.
14. Course Registration Form (THIS MAY BE PRESENTED LATER IF NOT YET ONLINE).
15. 12 Coloured Identical Passport Photographs.
